The Carolina Disc Golf Club of UNC-Chapel Hill - PID/Password Creation

Since some members of the club are not affiliated with UNC or joined prior to the creation of the My Stats page, some of you do not have a PID or password stored in our database. This form allows you to create one for yourself. If you are affiliated with UNC, please use your UNC PID as your password, as we need this to be an officially recognized sports club.
